With 30+ years of nursing experience, I have enjoyed a profession that demonstrates what it means each day to truly care and deliver the best possible service in very challenging medical situations. What I have learned is that when you teach patients how to care for themselves and exercise their own agency, they will have better outcomes. That is, I watched people utilize the emergency room for primary care needs, which prompted me to seek an advanced degree and become a family nurse practitioner. This decision changed my life as I attained additional knowledge and skills to be more impactful to the community.
With confidence and intention, I was on a journey to care for the unhoused population in Greater Philadelphia. This goal brought great joy to my heart. By establishing Solutions 4 Home Care, I can better teach patients the importance of maintaining their health. I encourage them to not only take their medication but also to learn about and utilize healthy choices. I ask them to be curious about their own health and to search for examples of how best to care for oneself. I share with patients my cases and stories that relate to their situation. I feel it is critical for patients and their caregivers to see how outcomes can change for the better. It is also important to note that I am very grateful for the supportive colleagues at Homecare MD, a leading California primary care organization where I enjoyed years of service. Their leadership team gave me insights on how to provide exceptional care to patients in their home in partnership with homecare companies. We both recognize the value of the social determinants of health and health literacy frameworks to improve the overall outcomes of our patients.

Identifying polypharmacy concerns helps patients better understand their disease process. We perform mini-cog function tests at the start of care and resumption, which can predict the patient's outcomes and will support the discharge function scoring.
